open-iscsi use of multi-queue to implement MCS

The open-iscsi package is the de facto standard Linux iSCSI initiator, but it does not support iSCSI MCS.

MCS stands for multiple connections per session, and it means that the iSCSI initiator and target communicate over multiple connections to increase throughput. It is sometimes written as "MC/S".

The current SLE 12 target is LIO, which supports MCS. We don't want that other non-unix company to have the only initiator that supports MCS.

Note: I do not expect to get very far on this, but I'd like to start the investigation.

      I've already exchanged some email with Mike Christie about this, so first step is to review those. I will post an update when I do.
